The AUS HEARTS team is starting Phase 1 of our 5-year project to adapt, implement and evaluate the effectiveness of WHO HEARTS to improve the early detection and effective management of hypertension in regional, rural and remote Australia, where hypertension rates are highest.
Phase 1 involves understanding what the current practices of hypertension management are in Western NSW, to inform how the evidence-based model can be implemented feasibly.
Tim and Katrina, Health Service Engagement Officers for AUS HEARTS, will be reaching out to primary care services over the coming weeks, to invite your practice to be involved. Phase 1 will involve completing a self-assessment checklist. For taking part, you will be given an evidenced based quality improvement feedback report and compensation of $125/hr.
